Description
This 68-year-old patient with a history of left breast cancer treated with mastectomy, radiation, and latissimus dorsi flap reconstruction presented with noticeable asymmetry and contour irregularities. To improve balance and achieve a more natural appearance, a comprehensive revision was performed. On the left side, the prior implant was removed, the capsule was released to correct tightness, and a new 325 mL silicone implant was placed to optimize shape and projection. The mastectomy flap was refined to address irregularities, and 30 mL of the patient’s own fat—harvested from the inner knees—was carefully grafted to enhance contour and soften transitions. Attention was then turned to the right breast, where a revision mastopexy (breast lift) was performed with excision of redundant tissue and reshaping of the underlying breast to improve symmetry with the reconstructed left side, along with placement of a 145 mL implant for augmentation. These combined procedures resulted in a more harmonious, balanced, and natural-looking outcome.
